Probably TMI for the guys on my flist

My doctor wants me to keep a detailed record of my menstrual cycles now that I'm officially peri-menopausal. Knowing how skilled I am at misplacing small pieces of paper, I made a html version of the chart. Being comfortable with simple html, I should have no problem updating it -- providing I can remember where I keep the working copy.

If any of you wound find such a chart useful, I have put a blank copy on the web. You can find it here. (Note: the form is rather wide and you may need to change the orientation to Landscape in order for it to print properly.)
